I'm not familiar with the LTM stuff, but being a rangefinder I would think it would need to be fairly precise to enable the coupling to work for focus.
This is not really an issue since it is the offset from the focus cam to the lens flange that matters and the focus cam is generally a full ring. Hard to describe, but fiendishly simple.

When I started using m42 lenses I was amazed at how closely they all lined up. It is very difficult to make a thread to start at exactly a desired point, and quite difficult if the 'lining up' is setable during assembly.

There are no extra points awarded for perfect alignment at the 12-o'clock position and no mechanical reason to precisely do so.

When setting up adapters on my M42 lenses for 'modern' bodies I intentionally calibrate them so the 'diamond' aligns at the 10- or 2-o'clock position. That makes it much easier to read the aperture data since it's not hidden under that #$%$# internal flash bulge. You can do that by shimming the flange-less adapters. As noted above, registration is based on lens base-to-body fit, not radial alignment.

Life was good with an M- or K-class lens on a K1000 or MX body without an internal flash.
