That's always been the way it is when I have purchased, and when in doubt the phone guys have always told me what is included. Manuals for the dSLRs are available online in PDF format too.

I would say that the ratings depend somewhat on the type of item. I have bought several lenses and both my Canon P and Yashica 44 from KEH and all were in appropriate or somewhat better condition than described. I have also bought a few view camera roll film holders, all described as Ex. They have been as described or somewhat worse. In fact, one was undeniably UGLY. They accepted the return, no questions asked. I would expect that any item that would have been part of a pro kit might be a bit more rough.
