Success!!!!

I finally found a shop that is able to get this for me/us. Unfortunately the price is a bit steep @ $200.00. It includes both the Ring/'Collar and the foot itself. Shipping a going to on the order of a few weeks as it as to come from overseas first then to Advanced then to you.

So here's the deal Contact Advanced Camera in Orego and Ask for Jennifer (Full contact info at the end of the message.).

You'll need both of these part numbers O-TM80 and DFA 150-450 Tripod seat assy 41610 0-D401, unfortunately you can't just get the foot.

Here's a message from Jennifer about getting these and other parts:
"As far as ordering for others in the Pentax community, we'd be happy to try to obtain parts. Small caveat being this: We were grandfathered in for parts ordering as Pentax/Ricoh dissolved their nationwide network of authorized Pentax repair centers. However, likely due to the small amount of parts we order and I suspect they are discouraging shops like us from ordering, we often find a lag in communication with them (obviously) and they do not help us identify/search part numbers for the parts we need. Also, for many older cameras and lenses, they no longer sell parts (eg., 645). Having part numbers is super helpful to expedite the process and expectations should be to a minimum. Having said that, however, we are happy and willing to try to help locate parts and thank you for passing our name along!"

Kudos to Jennifer and the crew there for their help in getting this when no one else was able to. Next time I'm in Portland I know one where one of my first stops is going to be.
