I have maybe six adapters, from Pentax to dirt-cheap imitations that required considerable modification to work. I remove the springs and keep the adapters mounted to the lenses. I use mostly small primes so none of them put a lot of stress on the adapter. I don't have any problems with lenses falling off unexpectedly. A stiff focus ring can cause trouble, though.

I would use a genuine adapter unmodified on a large heavy lens, or maybe make a hole for the locking pin. I have considered buying an adapter with a flange to help locate a locking pin hole exactly.

I've read some horror stories about M42 adapters that are not sturdy enough and one can easily drop either the lens or the body. Well, I just got my first M42 lens yesterday (a Macro-Takumar 50mm) and almost dropped it already. It seems the off brand adapter I have rotates easily even with the spring/clip so that one has to be very very careful.

As much as I like the lens, I already wish it was using the K bayonet.
