My guess is that the black dots were from the bleach stage. How are you mixing the A+B? If either parts are powder, then it may need better mixing. If not that, then it goes back to the bleach agitation. Before it was too much too aggressive, and here perhaps not enough too little.

@Not a Number below has a good point. Could be air bells in the bleach step if you don't tap the tank sharply to remove them.
