I've had this issue as well, but always attributed it to over-tightening on my part. The screwdriver works well, but really I just need to remember to take it easy when putting it on.

managed to get my original grip off today by wiggling the grip a bit and then just turning the wheel by hand, luckily didn't have to resort to a screwdriver because I would surely end up scratching either the camera or myself

The connecting pins were all ok.

Learned my lesson and now tightening only finger-strength-tight, should be enough.
