I have had one come off when it was going into a bag that was a tight fit. They are generally tight enough to stay on but loose enough for when you want to switch it for the magnified eyepiece

It's a very tight fit on my K10D. The rubber cushion tore rather than the whole eyepiece slipping off in a camera bag.

The design did change a bit.
Some come off easier, some sit better (K3 which is slightly different)
But my three O-M53's sit on all their bodies perfect.
I only use those.

These just slip on and off. I lost one once, had to track back to find it. I've bought some third-party replacements that I use for film cameras I buy that don't have them when they arrive.

Manufacturing tolerances.

Some will be a little looser than average, some a bit tighter than average.
