RRS makes lens collar replacements. Best to send an email to their Customer Service or Tech Department. Explain what you are trying to do and give them the diameter of the lens. They may be able to recommend a particular replacement that they have in stock.

This is a very helpful thread. Thanks to all who posted.
I bought a Precision Design collar for Canon 70-200/4 L from Cameta Camera.

Precision Design Tripod Mount Ring Collar CA(W) for Canon EF 70-200mm f/4L & f/4L IS USM

With a thin cardboard shim, it fits nicely on my Tamron 70-210/4 (19AH). My only concern is that the collar lock knob is pretty rough unless the shim is the absolute minimum thickness (knob mates closely). Other than that, the construction is good and mates well to the Acratech 2.5" plate.

The concept of using insert adapters to match tripod collars to lens barrels is the trick to satisfactorily using aftermarket tripod shoes.

Some thoughts:

The adapter doesn't necessarily have to completely encircle the lens; three or four smaller pads may be sufficient. They don't even necessarily have to be symmetrical! This can prove useful when when the collar tends to obscure some important part of the lens. I've successfully used the small felt pads such as sold for eye glasses as fillers and pieces cut from a leather belt.

Likewise, the adapter doesn't necessarily have to be the full width of the collar throughout the complete circumference of the lens if there isn't a wide enough space on the barrel to prevent interference with movable aperture/zoom/focus rings.

Adding a palm rest to the tripod shoe greatly facilitates hand holding longer lenses; it allows the hand to support the weight while leaving the fingers free to focus and zoom.

An extender can also help find a good balance point when the collar doesn't attach at better location.

In some cases the collar can be place so as to act as a handy zoom stop.
