The carabiner is metal, and has a screw-down collar which prevents accidental opening. Although I do back it up as described above, I have never had it fail. It's more likely (but still rare) that the tripod thread loosens - when I'm on the move I intermittently check it's still tight.
I also have the PD sling, It works brilliantly with the 645Z using both tripod threeads but I can't get it to sit comfortably with the K-1 and a big lens. It's fine (great even) with the KP and a limited, on which I use both strap lugs rather than the tripod thread.
