Any suggestions for a small bag?

I wonder if anyone has a recommendation for a small bag; I only need it to hold a K2000, a pancake lens, a 35mm macro, and maybe spare batteries.

I can take it all shopping with me, but the selection locally in shops is abysmal.

Ordering online feels like a "measure and guess" proposition.

Take a look at this Domke bag: Domke | F-5XB Shoulder/Belt Bag (Black) | 700-52B | B&H Photo
I use one for my DS and another lens or two. Very compact and easy to handle (like holding on your lap when eating, etc). Just cut the Domke logo off of the flap to make it a bit more inconspicuous.

I like LowePro bags. They don't shout "expensive camera gear" as some others do, they whisper it. I have a LowePro 100AW sling bag.

I get a K10D with grip and DA*16-50 mounted with DA* 50-135 in a separate division of the main pocket. (I removed all but one L shaped separator from the bag.) I put my DA 12-24 in the top pocket and sometimes the M 100/4 macro as well - a tight fit, but possible. Polarizers go in a front pocket (72 mm and 77 mm sometimes the 49mm for the macro).

I should have bought the 200AW, but then I would probably add more stuff (and weight).

If you want the smallest possible, I suggest ThinkTank's Digital Holster 10. Will hold all of that in a breeze and is very compact and exceptionally well made.

Holy cow, that digital holster looks like you'd carry a sidearm in there!
Utah probably requires a special license for it.

I'm surprised nobody has mentioned it yet! Crumpler 5 million dollar home. I actually get compliments on mine before people know its a camera bag. Which is half of the awesomeness - it doesnt look like a camera bag; which makes me worry less about it. The downside - it will really only carry an extra lens and a flash, and that's pushing it. You could step up (or down) either way in the Million Dollar home series, though.

I like the crumplers a lot.
I grabbed a small Lowepro on ebay for $5 and $5 shipping. I think it'll work.

Have you looked at Domke? Domke Bags
They're about the best I have ever used.

I have the Domke F-5XB bag and while I like it okay I find the flap/zipper combo annoys me. It is probably just the size/design of the bag but for some reason I feel like the zipper is too big or something and is kind of in the way. Of course, I'm weird so YMMV!

ANother vote for Domke. Have a look at the F6 great bag and you can change inserts to accommodate snugly different configuration.
