Thread thumbnails - how does it know?

I've been curious about this for quite awhile:

How are the thread thumbnails (desktop, not mobile) chosen?

Why do some change and some don't?

They are by default the first (retrievable) image that is posted in the thread. However, if the OP manually edits a post that contains an image, this image will replace the existing thumbnail.

I see - so if the OP does not edit a post, why would the thumbnail change?

If there are no edits then the thumbnail shouldn't ever change (though note that a moderator edit would count as an OP edit as well).
