Could someone explain what mode the camera and the flash was on including any specifi settings? I tried reproducing this with no luck, also on a K20 but with an M42 adapted lens instead.

You realize that with an M42 lens (or M and K series lenses as well) , there's no auto flash control from the camera? Unless you manually change settings on the flash, it just fires at full power.

You would have to be in M mode on both the camera and flash and adjust the flash output power as well as the lens aperture to compensate for the light required in the given situation.
