It looks out of focus but it may also be camera shake as well
for portraits.
shoot in TAv mode
set the aperture about f2.8... 1.8 won't capture much of her face
to avoid camera shake, set the shutterspeed 1/60th or faster for a 50mm (rule of thumb is focal length should be around the same as the speed of the shutter for still shots) eg 135mm lens shoot at 1/125th or faster
the iso will be automatic
take a picture near a window during the day or in shade out doors
This was taken by a window you can ask them to look out the window more if you don't want the dramatic shadows
Kids sometimes should be shot faster than 1/90th because they are high octane

Her eyes seem to be oof a bit. Next time focus on the eyes, not her nose or mouth.

In addition to the previous comments about focus and camera shake, the white balance is off, she appears to have a green tinge on her skin. Either find the appropriate in camera setting for the light source, or shoot RAW and fix it in post

@Silver-Surfer nailed all the issues. Main problem is that you didn't have enough light. So while you could shoot at 1/20 and f1.8 to keep the ISO down to 400, in doing so you were pushing the limits for both the shutter speed and depth of field. And had you boosted either, you would have started to take a hit on the graininess. That's also why the off nature of the white balance is so noticeable - because you probably only had one or two bulbs casting light on her and so the color tinge becomes so apparent.
