Could something like a rotating flash bracket help: Vello Speedy Camera Rotating Flash Bracket CB-200 B&H Photo
I've got one of these which allows me to hold the camera with the wooden grip, keep the flash high for no redeye, and then flip the base to put the camera in either a vertical or horizontal position. Then just use a cable release to trigger the camera.
If memory serves me well, you can even detach the flash bracket to make the rig more maneuverable.
