If it still shows up after a format, go buy another good quality card. The card may be defective that a format won't fix. I'd recommend an Extreme II or Ultra II as the best cards to own.

Well, it is a 16 GB Ultra II card, but I stored too many pictures in it. One more thing, sometimes, I took the card out without stopping it on the computer, maybe that was the reason.

I think Peter Zack is right, it looks like a writing error on the memory card.
