What do you mean by decentered? My 16-50 was sent to Japan to be "re-aligned" I assume you're referring to the same thing? The effects were, one side of each image would be out of focus in every shot, with a blue ghosting around objects on one side.

Indeed, but I still think f/2.8 shouldn't produce the image seperation seen in the previous images. I'd be interested in seeing a similar test with the same equipment.

I retook the image at ƒ/5.6 as c.perkins recommended. The result is much better and is leading me to think I have unrealistic expectations of clarity at ƒ/2.8 :-(

The below image has a few adjustments (crop, lens correction, levels, sharpness)

Here is the untouched jpeg

Thanks again for the responses.

Yes the phone camera has applied a healthy dose of sharpening, however one would assume with a larger sensor and a better lens the SLR should be able to capture more detail. If I apply sharpening, lens correction and some exposure adjustments to the earlier PEF, the detail still shows hazy on the paper pattern.

Will do some more shooting today with same settings as older product shots to compare. I'll also try changing the aperture. Will report back later

Thanks for the responses.

The focus point is on the die, so yes I agree the camera is front focussing and I'm familiar with the micro AF adjustment settings. What I'm more concerned about is the clarity.

To show the true clarity of the paper here's another shot & detail taken with my camera phone (Pixel 2 XL ƒ/1.8 1/120 sec 4.459 mm ISO242) Of course they're different cameras / lenses, but there's clearly something wrong with the SLR.

Ok I did another image, same settings but with the envelope taped down, still not 100% flat but much improved. Take a look at the detail image, there's definitely something amiss.

It's almost flat, it's an envelope to package a product so it has a slight bend in the top. I picked it because of the close black & white pattern, not the most scientific method but by my logic some part of each image should be crisp & in focus.

I recently sent my K-3 II and two lenses into my local Pentax repair centre for a checkup regarding a focus problem which I posted about last year, images weren't as crisp as they used to be, not super serious but certainly not desirable for the product shots I use the camera for. The diagnosis was that everything (all three items) was out of whack  The SMC Pentax-DA* 50-135mm F2.8 and K-3 II body were serviced locally, however the SMC Pentax-DA* 16-50mm F2.8 was sent to Japan, apparently requiring a more comprehensive alignment. I've received all the equipment back but the images are still lacking clarity. Before taking it back to the service centre I'd like a second opinion about what's going on. Can someone take a look at the photos below?

I'm no lens expert but I think the below images should help reveal the issue.
All were taken with my K-3 II body at F2.8 — ISO100 — 1/15 sec exposure. Taken by remote trigger on a tripod with autofocus point set on the 6 side of the dice in the image. 3 images per lens, long, mid and short zoom. No image processing done except export from PEF to JPEG. Paper background is 100% black and 100% white.

SMC Pentax-DA* 50-135mm F2.8 - 135mm
SMC Pentax-DA* 50-135mm F2.8 - 85mm
SMC Pentax-DA* 50-135mm F2.8 - 50mm

SMC Pentax-DA* 16-50mm F2.8 - 50mm
SMC Pentax-DA* 16-50mm F2.8 - 31mm
SMC Pentax-DA* 16-50mm F2.8 - 16mm

Thanks for the suggestion regarding the rear AF button. Regarding my original post, I tried all troubleshooting steps to no avail. I'm sick of not being able to rely on the camera so sent it in for a service. I'll update once I hear from the service centre.

Hey, thanks for the detailed replies everyone and sorry for the late follow-up. I'm running through the suggestions now beginning with cleaning the contacts on the camera & lenses with isopropyl alcohol. I'll report back in with results.

I'm not sure how or when this issue popped up, but my Pentax K3-II can't focus correctly anymore. I tried on both my lenses and encountered the same issue:
DA* 16-50mm
DA* 50-135mm

Following advice elsewhere on Pentax Forums I adjusted the AF fine adjustment to +10, which almost has it right, however I've noticed that when shooting at f2.8 the picture seems hazy, especially towards the edges of the image. Even at f22 the image isn't completely clear. Here are a couple of sample shots:

Is this something I can fix myself or a service centre job?
