7-Zip for Linux should be able to extract the files from the EXE version. Or the Linux zip command should strip out the zip file:
zip -F file.exe --out file.zip
or
zip -FF file.exe --out file.zip

Common errors preventing updates:
not placing the .bin file in the root directory of the card
firmware for wrong camera or file renamed when extracted. Check that the filename matches that listed on the Ricoh download page. If it doesn't match you either downloaded the wrong version or the extraction utility renamed the file if there was an existing copy in the same folder. These will usually have a number appended to the filename such as (1) or _1. Remove the existing duplicate file and rename the freshly extracted file.

Yes, you want the M or A versions, preferably the "A" for reasons stated above. I have the M and you have to move you eye around more to see the bottom of the viewfinder. Being an eyeglass wearer I'm used to doing that anyway.
Apparently according to a previous post you can enlarge the opening on the plate of the Ref Converter M and get the same view as the A. I haven't done it as I'd probably still have to move my eye around due to eyeglasses.
The A usually goes for more than the M on eBay.

So far all the "flagship" models from the K-7 onward and the KP are using an aperture control block based on a stepping motor. This is evidenced on the APS-C camera bodies as the cylindrical bulge between the grip and on the lens mount housing.

I find three-way heads better for what I normally shoot. It's easier to control one axis at a time.
A ball head can be faster because you don't have to lock/unlock three controls but can be a pain if you only need to change one axis such as shooting a panorama.
I just re-greased my three way with a heavy viscosity damping grease. What a difference it makes. I have a fluid effect on all three axes now. I suppose one could use the same grease on a ball head.

Specks of dust or debris in/on the lens would not make such sharp images. Odds are you sensor requires cleaning.

I would put a piece of wood or something between the lever and the slot wall before attempting to straighten the lever. That way only the bent part will move and not someplace further in the lens - or even snapping off the lever.
The bent lever may indicate the lens was dropped without a lens cap. Inspect for other signs of damage. Or return the lens and find a better copy.

I would hazard to guess it is the lack of parts more than anything else affecting serviceability. If the parts aren't available, they can't sell them to end users no matter what the policy is.
I wanted to order some parts for my LaserDisc player but the part are no longer available on Pioneer's website. They were still available four years ago.

Most likely place to look is the light seal at the door hinge side as that is where the take up spool is. The film canister should block light on the opening side of the door.

Since the body appears to have been sent to Precision/Ricoh we shall have to see if the problem can be sorted out. It sounds as if the equipment is still under warranty or extended warranty.
Cross flashing the firmware shouldn't happen without modify the firmware binary files or flashing via external hardware. It seems incredible that the wrong firmware would be installed at the factory.

I would use a "flanged" adapter to connect my M42 bellows to K-mount body as the genuine Pentax adapter has some rotational movement no matter how tightly I screw in the bellow adapter. Since the flanged adapters use the standard K-mount body locking pin I'm guess most of the rotation can be eliminated. The extra few millimeters aren't going to make much of a difference with bellows.

The Chinese adapters are ok if you remove the screw and clip and screw them onto the lens semi-permanently using threadlock.

The weight of the lens should not matter as there should be no movement of the adapter that would compress the spring clip allowing it to turn enough to release.
The spring may be loose, bent/compressed, worn or modified so that it doesn't lock the adapter from turning.
Photos of the adapter particularly of the spring would be greatly helpful in trouble-shooting.

Definitely an OEM adapter. Allowing for perspective the spring appears to be the same as all three of my adapters. 1 PENTAX JAPAN and 2 PENTAX Philippines.
There are two ways to install the adapter depending on which set of instructions you have:
Line up the red dots, insert and turn clockwise until it stops. You might hear a faint click as the spring engages.
or
Screw the adapter onto a lens, lettering pointing to the lens. Then mount as you would a K-mount lens, lining up the red dots, insert and twist clockwise until it stops.
Either way the adapter should stay in the camera when you unscrew the lens. The spring needs to be pressed in towards the center to turn the adapter anti-clockwise for removal.

OP says that it is a Pentax M 135mm f/3.5 lens, which I would think is highly unlikely to have issues with the power contacts in any Pentax body. Otherwise other owners of M series lenses would have noticed this incompatibility years ago.
There is probably some other damage to the lens as well either with the aperture lever (does it move freely with very slight resistance from the spring?) or to the bayonet tabs.
Did you own the MX body or did you buy it with the lens?
Does the lens fit on another body or other K-mount lens fit on the body without problems?
Return it for you money back if it wasn't sold as for parts only as "not as described".

If the ring is used the iris should not close down more than the ring setting. If it does then the aperture mechanism is defective.
Remove the lens from the camera and turn the aperture ring off the A setting. The diaphragm should open and close with the ring. If it doesn't then the lens needs repair.
Is the aperture actuator lever intact and working? Set the lens to A or the smallest aperture (F22), dismount the lens and move the lever at the rear of the lens. Does the aperture open and close. Is the lever there or has it been cutoff by a previous Canon owner?

KEH cannot get parts as noted by several previous posters, so it will probably be a waste of time (and money if you send your equipment to them for an estimate) contacting them.

Cold will effect the efficiency of batteries. Silver Oxide cells have a much flatter service % drop off with lowering temperatures than alkaline.
