I assume that picture shows you were looking into the lens from its mount side. If so, that defect appears on one of the front lens elements. That may be lens separation. I say this because I used to have a copy of this lens that had the same problem at that side. I didn't notice it at first. But then I found the pictures I took with it lacked contrast, and some times were washed out. So, I looked into the lens against a light source and saw the lens separation. If you check the lens diagram here: SMC Pentax-DA 18-55mm F3.5-5.6 AL WR Reviews - DA Zoom Lenses - Pentax Lens Reviews & Lens Database, the second element from the front end is an aspherical element, which is glued with another thin element. The separation could be happening there. I think that was the case for my copy. It is likely in yours too. You can take some pictures against strong light source, and you will see the pictures are washed out and lack contrast. If so, it proves it is the lens separation.
So, returning the lens is your safe bet.

Remove the film and re-install the insert. Press the shutter release button while depressing the Mid-roll Wind-up Button. See if it can get rid of the "E".

Are you saying that piece on your copy has a blemish? Or are you thinking that piece itself is a blemish? The picture you show is the way it is on the Pentax lens. My copy is exactly as what you show here. Pentax version doesn't feature a switch there, therefore it has that small piece to cover the area.

The mirror has tilted on K10D over time. Not only this - you probably will notice it is hard to frame squarely since the mirror is tilt relative to the mirror box and the focus screen. It is a common problem on K10D. Google it and you should find many such reports. K10D only has one support point under the mirror. Some found it helps to address the problem by attaching a rubber piece to the mirror box under the unsupported side, but the size of the rubber piece must be just right.

If only pan the camera, the double pan ball head will keep the level. The top panning control is for this purpose.
But, I'm not sure if this was OP asked for.

@Coven
Mine behaves exactly the same as yours. Focus ring is harder to move and less smooth than zoom ring. I bought it new and have been using it for 10 years. It works fine. Hope this helps.

I recommend Pentax AF280T. It is more than sufficient for a film KM. Its manual controls are the best match for KM. It is powerful and build quality is high. It is dirt cheap nowadays.

My understanding is: Pentax MZ-6/MZ-L/ZX-L fully works with F, FA and DA lens. It also works with D-FA lenses as long as they have the aperture lever (instead of electromagnetic aperture). Electromagnetic apertures are only on latest lenses such as D-FA 50mm 1.4, D-FA 85mm 1.4, and maybe others.

Maybe battery is weak? I remember seeing the two LED dots got weak when it was time to replace battery.

Sorry to hear. Judging from your pictures, yours looks exactly the same as mine. The button has a pin inserting into the hole on the adapter. The other end of the pin (inside the adapter) is larger than the pin itself. I guess this is to prevent the button from falling out. The pin directly pushes against the lever. I don't see any other mechanism. On my adapter, if I move the lever from inside, the button/pin can slide easily through the hole. I think your adapter may have rust at the pin-hole connection. Good luck!

That I don't know. You need to make the lever move by pressing the button - this is how lock/unlock work. If it doesn't work, you could try lubricant.
