Australia oriented model or photo release (model under 18)

As well as photography I also write as a hobby, have published my first book and am writing the second.
I took a photo last year that would make a good book cover and the young lady, and her parents, have consented to its use. A figure has been agreed upon and all I need is a model or photo release for its use.
Google on this occasion has not been my friend, there are a ton of forms of varying legal complexity, many reference a particular organization, such as Shutterstock, as part of the release and oddly, many reference " the consideration" for the fee offered with no provision to include that fee other than just scrawling it in somewhere, and many only allow space for one parent to sign and not both.
If anyone can reference an online form that would be great, otherwise I'll sit down and type up my own, which is starting to seem the easiest option. The usage is on an ebook that will go on Amazon Kindle, there are no plans to ever do a print version.
And just to clarify, since the Photo has already been taken, is it actually a photo release rather than a model release that I need? Thanks,

Thanks to steephill for picking me up on Delta not coming to market till 1990. I was scratching my head wondering how I made such a howler after trying to research this properly, all I can think of is that I distracted myself going through some brilliant galleries of Delta shot images and then totally forgot it was the wrong era.
Will be having fun with this working Ilford Cibachrome into the story, I had a number of prints made on this and I adored the results it gave, dearly wish I still had them now.
The input of everyone is massively appreciated, now all I have to do is hope that someone is interested in a story about a UK gymnast who becomes a model and her adventures, both in front of the camera and elsewhere and I actually get a few sales on this.
Hell of a lot of fun writing it anyway.

Back at this time i was using Agfa 50s Professional, great film that I enjoyed using a lot.
I'm writing a short novel that references, in some detail, a nightime photo shoot involving a sports car, a model and a very long flowing fabric train. Although it's not strictly necessary to be 100% accurate, I'd like to keep it fairly factual and I'm wondering what colour slide films would have been the "go to" in 1983 for such a shoot.
For Black and White I've referenced Ilford Delta as the film of choice, Agfa 50S is one of the colour films he will be using, but what else, probably of a higher ISO rating, would have been a good choice?
On a tangent, saw a great movie last night, ANNA, which was a box office disaster and has an awful trailer that no doubt helped kill it. Despite this it had plot twists around every turn and I loved it, despite the action scenes being horrendously implausible. What was fun was seeing the depiction of the model fashion industry and it's photographers, (this was set I assume in the early 90s) the latter so unrealistic as to be hilarious. Was curious to see Mamiya as the brand of choice for all the photographers shooting, I assume, medium format, and I'm wondering if this would also have been one of the cameras of choice for a landscape photographer in the early 80s?

Huge understatement, had a brilliant time, loved doing stuff out of my usual Cosplay, met a great stack of people and would love to attend something like this again.
Some street photography the next day around Angel Place before heading home to Forster, couldn't be happier. Now if only a Pentax mug and strap had been in the gift goodies.....

And if anyone wants to have a go at fixing this it will be great. Essentially after it being lightened a little on her face LHS, regaining a little detail but without losing the night time moodiness. It was shot under hard light at ISO 1000, the skin looks a little grainy and could use a bit of smoothing, preferably while still keeping a bit of detail, and if the eyes could be lightened a little to have a touch more impact. Otherwise happy for anyone to do it however they reckon, always interesting to see someone else's take on how a photo should turn out.

I attended the Pentax 100 year celebration in Sydney a few days back, had an absolutely brilliant time, and I'm really happy with my shots considering that I don't do night photography under lights. I have one shot though that I've fought with it, can only get it so far and I'm considering sending it to a professional and see what they can do with it, I really feel there is a better photo lurking in there for someone with a better editing skill set than me. It will also be interesting for me to be able to compare my edit against someone with a real skill set.
The shot is only for my use so I don't want to spend much on having it done, probably $30 Australian maximum, preferably less. A straight Jpeg of the unedited shot is attached, the original is 16mb raw file, and yes, I know there is a lot wrong with it. Still, I'm really interested to see what could possibly be done with it. I'm using Adobe Express at the moment, trying to learn Affinity, and having a hard time with it, it will take me a while to get good at it.
So, any recommendations for a photo retouched at the budget end of the market that is recommended?

Will have a group of photos up in another day or two, I very rarely shoot under lights, I'm usually out in the open at Cosplay events, so it was a great experience for me to do some night photography.

If anyone knows who the guy in the white shirt is it would be appreciated if you could let me know, I'm pretty sure he was one of the Pentax management team but i didnt get his name, same for the other guy as well

I attended last night, had an absolutely fantastic time and loved the event. I will be posting photos here after editing, probably in two days time. Thanks to Ricoh/Pentax , C R Kennedy & Co and Eventbrite for hosting. Thanks to the five models for posing happily for us and the convivial party crowd in general.
Really was worth the trip down from the mid coast for it, it was a great event.

I usually crop my images so editing usually has happened to images I put up, although rarely beyond crop and minor colour tweaks. Facebook compresses images that it deems too large and I'm wondering what that limit is so that I can avoid it. An advantage to Lightroom seems to be that you can set a maximum file size that it will stay under.

Since getting a K50 I've been shooting in Raw plus Jpegs at the 5 megapixel setting. The latter are only for Facebook and I'm slowly getting my head around Lightroom - not my favourite program but it does some things well, particularly resizing images.
I'm asking if there is any reason to shoot the JPEG in addition to the Raw file, or just let Lightroom resize the Raw and convert it to JPEG. If the latter what settings apart from sRGB should I use? Currently I'm setting 920 pixel along the longest side and a maximum file size of 100kb, but really have no great idea if these are ideal.
Facebook isn't the greatest for displaying detailed photos, but I'd just like to get the best online IQ I can. All comments very welcome.
