As others mentioned, with 55-300 you better use the DCR-150
and I tried it with the 18-135, it doesn't work! because of Internal Focus design of the lens.

DCR-150 on DA55-300 @ 150mm, the coin lighted with a colorful background on my cellphone screen. just cropped from sides to make for more squarish crop.

As mentioned in Raynox's website, the working distance (when focus is set to infinity) with 150 is 20cm and with 250 is 10cm! and as you change the focus to MFD the working distance will be shorter a bit, probably around 1 to 2 cm as audiobomber mentioned.

Here calculate it yourself. ;)
dcr-150 has a diopter of 4.8 and 250 has diopter of 8.

Yes, and then I've used the Raynox with 300mm lens, and know how hard it is to get perfect focus! specially if the subject is ant! which is one of the most active species I've tried to shoot.

Here is the calculator: Macro Extension Tubes & Close-up Lenses
With DFA100macro (At MFD) and Raynox DCR-250, you should be around 2.6X or 2.6:1 magnification.

Thanks, yes here when you walk in the park and look into foliage. there's so many small creatures moving around, but the sad part is that whenever I go there shooting them, mosquitoes do shoot me too. haha.

While Raynox DCR-150 doesn't give much magnification on a 50mm lens, it gives my DA50/1.8 good amount of close focusing ability to be better for flower photos.
Here are some of the shots with DA50/1.8 and DCR-150

Between f/11 to f/32 :D

There's no such a thing! it depends on the magnification and the amount of DoF you need. And when you are using a close up filter usually you want to have something around or even more than 1:1 magnification, and when you are there, you need to shoot at higher f-stops for more DoF.
A good thing about using a macro lens, is that when you feel the need, you can just step back and shoot from more distance and later crop the shot, this way you can have more DoF. but most of the times when you are using a Raynox or things like that, you are limited by magnification and movement.

The first one was very straightforward and easy, mounted the camera on tripod and fixed the fork in front of it. shoot with different angles and apertures. add some contrast in post.
The second one was tough, because I had to focus on the pouring water which coming out of water tap, but it was in different distances based on the pressure of water. thus it came out a bit OOF. (Have put a colorful board behind to add some color to the photo.)
