What I'm saying is that the people who are getting good results are using F/4 - F/8. They shoot where the DoF is as large as the subject. As I have said previously, There is latency in the system which is exactly what you are seeing. The AF locks, but by the time the shutter releases the target point as moved out of the plane of focus when using F/2.8. As you say, using F/4 - F/8 doesn't actually test the AF speed. As you point out Biz-Engineer is shooting his horses are F/4 - F/8 so that most if not all of the horse fits in the DoF. Its not really much of a test for the AF as you could employ zone focusing and get the same result on a manual focus camera.

You don't need 1/5000 of a second. 1/1000 should be fast enough to freeze the motion of your dog. The DoF at F/2.8 is going to be your problem. Most of the people showing good results are shooting at F/4 or even F/8. They aren't stressing the AF as much as you are with F/2.8 at 200mm. When the DoF is larger than the subject it a lot easier for the camera to get the head in focus. Change your setting to F/5.6 and give it a try. You will get much better results. The K-1 AF isn't going to give you great results at F/2.8 200mm.

You will find there is latency in they system. How much? I don't know, but your results are consistent with what has been posted before. By the time the AF locks and sends the signal to release the shutter the subject has moved out of focus. Even with a toddler walking towards you, you will get similar results. I keep hoping Ricoh will improve this with better firmware, but so far nothing.

Have you actually tried those settings? AF hold set to medium would actually make the AF worse under these conditions.
The wide area 33 AF point setting is slower than the single point or the 9 point. Ricoh is either using very slow hardware or very poor algorithm..... or both.

Yes. There is noticeable latency in the system. It is possible to shoot moving action like a race where you have predictable movement, but for dogs running dead at you, the AF is going to struggle compared to that of Canon, Nikon, Sony, Fuji, Olympus, or Panasonic. It is better than Phase One or Hassy though. Part of the problem is the D-FA* 70-200mm is not a really fast focusing lens. My Sigma 85mm F/1.4 is my fastest focusing lens and is noticeably faster than my D-FA* 70-200mm. I believe the 28-105mm is also pretty quick, but I have only used it once.

Did you ever get the AF issue resolved? I have been pretty outspoken about the below average AF performance of the K-1, but it is significantly better than the AF in the 645z that you have. Outside of tracking moving subjects in mixed and low-light it performs very well. For travel and landscape photography its probably the best DSLR you can buy.
